Job Description
Collect and assess data from the patient/family/caregivers and medical care providers to identify variables affecting care and initiate the development of an individual care plans and initiate referrals for identified admission and discharge needs. Conduct and document an admission interview with patient and/or significant other/care providers with no exceptions based on chart audit. Document the initiation of discharge planning and initiate referrals as necessary as evidenced by development of care plan. Exceed Standards: provide two chart audits documenting discharge planning. Collect and evaluate pre admission documentation and diagnostic testing results from multiple medical sources, patients and family. Collaborate with surgeons, primary providers, consultants and anesthesiologists to ensure patients are optimized for admission, educated and instructed appropriately. Assess, plan, implement an! d evaluate patient information exchange and/or education progr! am which will meet patient/family learning needs. Confer with nursing staff, physicians, managers,clinicians, patients and patients to assess educational plan needed for patient and family as needed. Implement patient/family educational plan as needed and develop strategies and methods appropriate for teaching the patient based on their individual learning styles, utilizing adult learning principles and the nursing process as evidenced by patient education documentation. Determine effectiveness of patient/family education utilizing methods of on-going evaluation (e.g. patient oral feedback/surveys, feedback from personnel, physician consultations and decreased number of cancellations). Serve as a liaison person for patients, medical staff and hospital personnel to expedite the admission process and impact the discharge process. Serve as a consultant to hospital/clinic staff regarding admission process. Collaborate with appropriate staff to facilitate smooth admission proces! s for inpatient and outpatient admissions or services. Assist a multidisciplinary team in developing, maintain and evaluating an effective admission process to ensure a streamlined, efficient system of admissions. Collaborate with a multidisciplinary team in designing a patient-focused, efficient admission system. Participate in role evaluation on an annual basis by identifying areas of strengths and limitations and setting goals for improvement and/or expansion. Practice cost effective use of self and supplies and suggest alternate approach on cost-effectiveness as appropriate. Conduct planned Quality Assurance programs related to the admission process as evidenced by completed documentation.
Basic Qualifications:
Experience : Minimum of three years medical/surgical experience required. Education: Graduate of an accredited school of nursing with current licensure as a Registered Nurse in the State of Minnesota required. Bachelor of Science in Nur! sing preferred.
License or Certification:
Licens! ure & Certification: Basic Life Support (BLS) certification required and Registered Nursing (RN) License in the state of Minnesota required.

Schedule Details: start time 0800 to 1000, currently no weekends
Weekend Schedule: currently no weekends but possibility in the future
