Job Duties and Responsibilities
There
are four services that comprise the bulk of our work in cardiovascular
monitoring: invasive line assistance and
hemodynamic monitoring, nerve bloc ks, cell savers, and intra-aortic balloon
pumps. Our role in the hospital allows us to be with a variety of patients, cases,
and various levels of emergencies.

One
of the procedures the CVMT is involved with is called invasive line
assistance. This usually involves the
CVMT assigned to make fluid setups as ordered by the physician. These prepared fluid lines are connected to
the patient so that the anesthesia team is able to directly monitor arterial,
central venous and pulmonary arterial pressures. During the actual procedure of placing the
invasive lines (this usually takes place in the operating room before the actual
surgery takes place), the CVMT assists the anesthesiologist by gathering the
appropriate supplies, handing supplies off in a sterile manner and operating
the ultrasound.

Prior
to the start of a patient’s surgery, the CVMT also can assist the
anesthesiologist with placement of nerve block s. Nerve blocks enable an
anesthesiologist to bathe a specific nerve in a local anesthetic which allows
for pain control following orthopedic procedures. The CVMT’s assist with nerve
blocks in a very similar manner to the placement of invasive monitoring linesâ€"whereas
the CVMT’s prepare kits, operate the ultrasound, and ensure all appropriate
monitors are in place.

During
the actual procedure, the CVMT may be assigned to operate blood collection
devices, such as a cell saver. Cell savers enable blood to be collected from a
surgical procedure, which can then be processed to red blood cells and returned
back to the patient. This reduces the need for transfusions of donated red
blood cells (RBCs). For this service, the CVMT’s typically monitors the machine
during the operating room case. If the
patient experiences significant blood loss, the CVMT begins processing the
blood so that it can be returned to the pa tient.

Two
devices that the CVMT operates outside of the operating room are the
Intra-Aortic Balloon Pump (IABP) and the Impella device. To over simplify, an
IABP is a device that reduces the workload on the heart while simultaneously
increasing cardiac tissue oxygenation after the patient has a cardiac event,
such as a heart attack. IABPs are placed by a physician (usually
emergently) into a patient’s aorta. The Impella pump also helps support the
heart by promoting blood flow from the left ventricle to the aorta and to the
rest of the body. These devices are frequently placed during high-risk
Percutaneous Coronary Intervention (PCI) procedures which can be scheduled or emergent. The CVMT
monitors and operates the functioning of both machines in the operating room and in cath lab, and after
they are placed and the patient is in the intensive care unit.

The
CVMT position is full time and requires a two-year commitment. Due to the rigorous three-month clinical
training program, it is our expectation that the new hire will commit to the
position and become an integral member of our team. After successfully completing the training
program, the new CVMT will be allowed to work independently with the physicians
and provide services.

The
majority of our caseload is done at Abbott Northwestern Hospital in
Minneapolis, but we also provide our CVMT services to United, Mercy, Fairview
Southdale, HCMC, and North Memorial, to name a few.
